Oh, no! Not the face!

Yeah, it's about that time.  We just got back from break on Monday, and we began a new unit in Anatomy - the Head&Neck.  Queasies, consider yourselves warned... I can't be highlighting entire posts, that would seriously cramp my style.


Anyway, with this region come many new and/or exciting things, like finding out what those glands in your neck look like, performing a cricothyrotomy (that's the procedure T.V. docs are constantly shown performing, with a ball-point pen and a knife, by punching a hole in someone's throat to give them an airway), and seeing the muscles that control a smile. 


This should provide a lot of interesting things to think about.  But I have to learn all the parts of the skull now, so for now, let's just say I owe you one.

Physcial Exam!

Today, we went to the hospital to play doctor and practice the physical exam.  A lot of my friends were nervous, worried they would mess up or forget how to do a certain test, or something, or the order of the tests, etc.  But there was really nothing to worry about, my preceptor at the site is very relaxed and just wanted to make sure we took a systematic approach, rather than making sure we found every little medical detail. Though I DID identify a murmur and make a diagnosis... that was pretty cool. Our patient today was in very good spirits, and I think that helped a lot. I t's fun!! It's like the history-taking course we took last year, only we get to actually DO things to/for the patient and relate to them better.   More to come, possibly, the next time we do this (a week from now). For now, we're watching some of the Planet Earth DVD. Ice Worlds.  You know, because of the blizzard and all. .

Internal Medicine Over

It seems like forever ago that I was in a rotation that was NOT Internal Medicine.  IM is, at 12 weeks long, the longest rotation we have in third year. It's not so much the length of each day that wears you [I should say, "me"] down, but more the sheer enormity of all those days, all in a row, all the time caring for patients with CHF and Type 2 Diabetes. But, finally, today we took the exam that heralded the end of IM clerkship.  This is kind of a big deal... it covers pretty much everything in medicine, since Internal Medicine is the sort of "gateway" into healthcare.  They're the ones who refer you to specialists when they're in over their head because you've got some weird infection from touching rose thorns. (Editor's Note: Quit doing that.) Now that this massively long rotation is just about over, I'm focusing on some relaxation techniques. Responding to the Bat-signal

I'm not typically a "theme" person, but this week has had a theme. I saw several patients that received borderline or frankly inappropriate treatment, and all share a common unfortunate thread. I already talked about patients who want surgery but don't need it. I also saw two patients this week who came in because they had gotten surgery that they didn't need, and now have problems. And another who kept coming back to the Urgent Care with what ended up being an abscess that was "lanced" (e.g. a tiny nick in the skin was made to allow the egress of some pus, and inadvertently trap the rest of it deep inside to continue to fester) and eventually needed to go to the OR for a formal debridement, which could've been avoided had it been done correctly the first time or had the patient been admitted to the hospital sooner.
